Implant Placement Techniques

  • Submuscular Placement: In this technique, the breast implant is placed under the pectoralis major muscle located between the ribs and breast tissue.
    The implant placed under the muscle provides support, especially for patients with a small rib cage and little breast tissue. At the same time, with the collagen structure formed around the implant, the edges of the implant and its own structure become imperceptible and provide the patient with a more natural breast appearance. Since the breast implant does not interfere with the breast tissue, it reduces the risk of capsular contracture and facilitates self-examination of the breast.

    Depending on the case, sometimes the upper part of the breast is placed under the muscle and the lower part is placed under the breast tissue. This technique is also called bi-planer. With this technique, the upper part of the breast is supported by muscle tissue while a higher volume is obtained in the lower part, which maximizes the breast implant shape to the natural breast appearance.
     
  • Subglandular Placement: In the sub-glandular implant placement technique, the surgeon places the implant between the breast tissue and the pectoralis major muscle. Since supramuscular placement is a shorter surgery, postoperative recovery takes less time. As the muscle is not intervened, there is no need for muscle recovery time. It is preferred by patients who are very active in their physical lives or who work in physically demanding jobs. This technique is more suitable for patients with sufficient breast tissue and muscle and is recommended for certain body types.

Implant Types Different 
Types of implants consisting of various materials are used in breast surgeries. We can list these implants as follows. 

  • Saline Implants: Saline implants are FDA-approved and biocompatible breast shells that are filled with saline fluid after being placed in the breast pocket. 
    These are placed underneath the breast tissue. Saline implants have advantages as well as disadvantages. Since it is filled with liquid during surgery, its size can be adjusted as desired and thus more natural results can be achieved. They also require smaller incisions than other implants. On the other hand, it is harder than silicone implants and may not feel natural to the touch. In case of any rupture and leakage, the fluid is absorbed by the body, but since the implant will be emptied, it may cause visible shrinking and asymmetries in the breast. 
  • Silicone Gel Implants: Silicone gel implants are FDA-approved implants that are pre-filled with cohesive silicone gel. Due to its nature, it mimics the natural breast sensation and is preferred because it is more natural in both appearance and touch than saline implants. In case of any rupture, it does not cause a deformity in the breast as the gel remains inside the implant shell. It is more durable than saline implants and is preferred because it can be used for many years.
  • Structured Implants: This type of implant, which is FDA and Canada Health approved, is filled with saline fluid inside the shell, which has an outer layer of silicone in order to look more natural and to be more durable than saline implants. It is actually a type of saline implant that acts like a silicone gel implant.
    It provides a more natural look and feel than saline implants, and also has the advantage of being able to be adjusted later, which saline implants have too.
  • Gummy Bear Implants (Form-Stable Implants): FDA-approved gummy bear implants, also known as Cohesive Gel Implants, are designed with a very strong and thick silicone gel that retains its shape even if the implant shell is damaged. Gummy bear implants are preferred because they have a high cohesion gel structure, which not only keeps its shape for a long time but also has a very low risk of leakage in case of fluctuation and any damage. This strong gel structure provides a more anatomically natural contour. It can be applied in Tear Drop (Anatomical) or Round shapes according to the patient's desire.

Incision Marks and Scars

Various types of incision marks and scars occur with breast implants. We can briefly list them as follows.

  • Inframammary Incision: It is an incision along the natural fold under the breast, also known as the inframammary fold, which provides the easiest access to the pocket to be opened for the breast implant, and this method is commonly used.
  • Periareolar Incision: It is usually opened in a half circle from where the areola begins to camouflage the incision marks for small implants.
  • Transaxillary Incision: With an incision made under the armpit, a path extending into the breast tissue is opened and the implant is placed here.
    It is preferred because the scar is almost invisible after recovery, but it is not suitable for precise implant placement.
  • Transumbilical Incision (TUBA): In this approach, the incision is made through the belly button and a long tunnel is created up to the breast pocket.
    Although it is easy to camouflage the scar, just like the armpit scar, the distance of the incision to the breast pocket limits the surgeon's control in the operation. Your surgical scar is always minimized by the surgeon in breast implant surgery. It will fade significantly after recovery.
